Family First Urgent Care performs physical exams for employment clearance and DOT or CDL licensing, scheduled by calling the office in advance.

Please call the office to schedule an appointment for a physical exam. When your employer requires a pre-employment physical or you need a Department of Transportation medical certification to maintain your commercial driver's license, you need an examiner who understands the specific standards that apply to your job. Family First Urgent Care performs work physicals and DOT or CDL exams that meet federal and employer requirements, documenting your health status, physical capabilities, and fitness for safety-sensitive work. We evaluate vision, hearing, blood pressure, range of motion, and medical history to determine whether you meet clearance criteria.


The exam includes a review of your current medications, past surgeries, and chronic conditions such as diabetes, sleep apnea, or cardiovascular disease that may affect your ability to perform job duties. For DOT physicals, our certified medical examiners assess whether you meet Federal Motor Carrier Safety Administration standards, including blood pressure thresholds, vision acuity of at least twenty over forty in each eye, and adequate hearing with or without correction. You receive a completed medical examiner's certificate or employer clearance form at the end of the visit if you meet all requirements.

What the exam measures and how it is documented

You arrive with a list of current medications, any medical records related to chronic conditions, and the physical exam form provided by your employer or the DOT medical certificate application. Our provider checks your height, weight, blood pressure, pulse, and respiratory rate, then performs a physical assessment that includes listening to your heart and lungs, testing your reflexes, and evaluating your range of motion in your arms, legs, and spine. Vision is tested using a Snellen chart, and hearing is assessed using a whisper test or audiometer.


For DOT physicals, the examiner reviews your history of insulin use, seizures, heart disease, or other disqualifying conditions and determines whether you qualify for certification or require additional testing such as a sleep study or cardiology clearance. If you meet all standards, you receive a completed medical examiner's certificate valid for up to twenty-four months, depending on your health status.


Family First Urgent Care does not perform physicals for school sports, immigration, or life insurance applications, as these require different forms and evaluation criteria. If you have a condition that may disqualify you from DOT certification, such as insulin-dependent diabetes or untreated sleep apnea, bring documentation from your treating physician showing your condition is controlled and monitored. We cannot issue a certificate if your blood pressure exceeds one hundred sixty over one hundred or if you do not meet vision or hearing minimums without corrective devices.

Most people want to know what to bring, how long the exam takes, and what happens if they do not pass before they schedule their physical.

What should I bring to my physical exam appointment?

You need a current list of medications, any medical records related to conditions such as diabetes or heart disease, and the exam form provided by your employer or the DOT. If you wear glasses or a hearing aid, bring them with you.

How long does a DOT or work physical take?

Most exams take thirty to forty-five minutes, depending on your medical history and whether additional testing or documentation is needed. You receive your completed certificate or clearance form before you leave if you meet all requirements.

What happens if my blood pressure is too high?

If your blood pressure exceeds DOT limits, you may be issued a temporary certificate valid for three to twelve months and required to provide follow-up documentation showing your blood pressure is controlled with medication. If it exceeds one hundred sixty over one hundred, you cannot be certified until it is lowered.

Why do I need to schedule an appointment instead of walking in?

Physical exams require more time than urgent care visits, and our providers need to block sufficient time to complete the exam, review your medical history, and fill out certification paperwork. Scheduling ensures we have the staff and time available to complete your exam without delays.

How long is a DOT medical certificate valid?

Most certificates are valid for twenty-four months, but your examiner may issue a shorter certification period if you have a condition that requires monitoring, such as high blood pressure, diabetes, or sleep apnea. You must renew your certificate before it expires to maintain your CDL.
